What We’re Looking For
Trillium Health Resources has a career opening for a TCL District Supervisor to join our team! The TCL District Supervisor is responsible for overseeing a skilled, multi-disciplinary team of professionals who provide localized support within a managed care environment to members who receive Transition to Community (TCL) and Behavioral Health services. The District Supervisor ensures staff implement industry standards, evidence-based and best practices to support members using the Trillium Software Platform. The District Supervisor provides oversight to ensure members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health/unmet health resource needs that help to establish positive outcomes for members and transform their lives for the better.
On a typical day, you might:
- Provide direct supervision for assigned teams of localized, multi-disciplinary Transition team supporting members with behavioral health conditions.
- Responsible for the oversight and assignment of Housing Slot Numbers.
- Ensure key transition activities are done appropriately and timely to include Pre-Quality of Life Surveys, identifying transition barriers, TCL member information matches across internal and external DHHS platforms, and final transition planning.
- Monitor DHHS Transition to Community Living Database to ensure required actions are taken as transition activities occur.
- Responsible for confirming monthly transitions, housing separations, bridge housing placement, reasonable accommodations, and enhanced bridge for direct reports.

Qualifications
Required:
- Bachelor’s degree or licensure as an RN, and five (5) years of experience providing care management, case management, or care coordination to complex individuals with I/DD, TBI and/or MHSU;
OR
- Master’s degree in a human services field or licensure as an RN and three (3) years of experience providing care management, case management or care coordination to complex individuals with I/DD, TBI and/or MHSU.
- Must have a valid driver’s license.
- Must reside within Trillium’s North Central, South Central, or Southern Region, which includes the following counties: Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, or Scotland.
- Must be able to travel within catchment as required.
Preferred:
- Work experience focused on supporting members with MHSU and IDD and transitioning members to permanent supportive housing in the community.
- Experience supervising a team.
