The Center Nurse plays a pivotal role in creating a safe, supportive, and enriching environment for adults attending the program, contributing to their overall health and quality of life.     KEY RESPONSIBILITIES

    Including but not limited to:

    Member Assessments: Conduct initial and ongoing assessments; develop and implement care plans;

    Medication Management: Administer medication to members and/or monitor self-administration of medication by members; monitor and record vital signs; may include insulin injections;

    Member Liaison: Work with members’ primary care physician and other medical team members; act as a liaison with family, significant others, and care providers;

    Family Communication: Foster open communication with family and care providers to understand and relate changes in members’ health status;

    Personal Care: Supervisor staff members providing personal care; provide personal care when needed, including showers;

    Documentation: Accurate and up-to-date maintenance of members’ health records, including progress reports, medication administration, nutrition, and any other documentation required by the local or state regulations;

    Bedside Manner: Maintain a compassionate bedside manner for members;

    Emergency Response: Be prepared to respond to medical and member emergencies promptly and efficiently; handle unexpected health issues and coordinate with emergency services when necessary;

    Community Engagement: Foster relationships with local organizations, referral sources, and family members; Provide caregiver and community support via educational seminars and events;

    • Possess strong leadership and supervisory skills to effectively direct and coordinate the work of clinical staff

    REQUIREMENTS

    • RN licensed in the state of North Carolina
    • 3 to 5 years of experience with assessment and treatment planning in adult senior care population
    • Monday - Friday, 8am - 5pm
    • Successful results of background check, and where applicable, additional North Carolina state specific regulatory requirements
    • Knowledge and experience with dementia-related conditions strongly preferred
    • Prior supervisory experience preferred
